Add "Activity Date" column to Faculty Forms page

When faculty go to the list of forms that have been assigned, they can see the title of the activity and the date the form was assigned, but not the date the activity occurs. For people who are assigned to multiple activities, this can get confusing, especially for anything that recurs or has similar names. It would be useful to have a column to shows the activity date so that faculty can more easily recognize what activity a specific form is attached to.

I agree with this. It would be helpful to show this vs auto populating the date of the activity in the form when sent. Right now, we have to notify the speakers separately the date of the activity to fill in their forms in anticipation of them not remembering correctly.

It would be great to be able to assign a due date that appears, so they can prioritize forms.

Agree to all of the above. And, if someone knows of a way to assign the same form in the same conference for peer review purposes I would love to hear from you. We recently had two different presentations that needed to be peer reviewed for the same conference and the reviewer was the same. I had to create and external form for the second presentation. I suppose I could create a Peer Review II form, but it would be nice if you could assign it more than once based on time/date of presentation or title.

That’s a good point, too! We have a mitigation form, too, but it’s filled out by CME staff, not the person with the relevant financial relationship, so if there’s more than one mitigation form required, we have to track that elsewhere.
