We’re exploring a way to follow up with learners about 30 days after they complete an activity evaluation, specifically those who indicated an intent to make a change. The goal is to capture practice change data using a brief 1–2 question form sent via an email reminder.
To keep the process as simple and accessible as possible, we’re hoping to avoid requiring learners to log in to our LMS to complete the form.
I’m hoping to get some guidance on a few points:
- Is it possible to host a form on a course page that does not require user sign-in?
- If the form is accessed without login, what options are available to associate responses with a specific activity?
- Is there a way to use a single form that can map responses to the appropriate activity, rather than creating separate forms for each activity?
Any insight or examples of how others have approached this would be greatly appreciated.
Hi Anita, here’s some answers to your points.
1- You can create a form that doesn not require people to log in, but it would have to be outside of the course
2- You can do two things that I can think of, you can either create a different form for each activity, or you can create a hidden element with the activity’s name that participants are unable to see. You can also make it visible and have the participant select the activity they participated in from a drop-down.
3- It depends. If you’re making this form accessible for activities that are not taking place at the same time, you can use that hidden element I mentioned above and change the name of the activity as needed. If you need the form for multiple activities taking place, you can make the element visible and either have participants write the name of the activity, or select it from a drop-down. I woud assume this can become bothersome as that would require a lot of babysitting. It would be much easier and faster to clone the form and give it a new relevant title in this case.
I put in a similar request a while back. We would like to see a feature added that would send follow-up survey’s… 30, 60 90 day. And we would like the ability to have the follow-up send based on particular responses (ie - yes to I intend to make a practice change based on the information learned in this session).
This has been our workaround (in this particular case we have a parent course with multiple child courses for an RSS). There’s probably a way to do this within a single course, but I haven’t tested this particular workflow in that instance.
- We wanted additional feedback from two of the sessions, so we created two hidden courses in the parent course outline (see screenshot) and added the survey questions in these course outlines.
- We created a reminder based on the learner being enrolled in the course that contained the survey link and message about follow-up and the need to sign in.
- We enrolled (turned off enrollment notifications) only the learners we wanted to follow-up from.
I realize this is a bit of work. We wanted this to live with the series so that it was easier to track.
